Age Requirement & Licensing
No tattoo services are provided to anyone under 18 years without a notarized parental consent form (DH 4146).
No tattoos are performed on persons under 16.
Intellectual Property & Photo Release
All images, artwork, text, and designs are © Duk Tattoo Corp. Clients grant Duk Tattoo the right to photograph finished tattoos and use those images in portfolios, social media, and marketing. If you object, inform us in writing before your session.
Booking, Deposits & Cancellations
- To confirm your appointment it requires a $200 deposit that will be deducted from the final price of your tattoo.
- Deposit it’s not refundable and to reschedule with no cost please let me know with 3 days in advance.
- After 3 times reschedule I’ll need a new deposit to book a new date.
